For insurance purposes, Dr. Myra Burgee is considered an “out-of-Network Provider.” This means that clients are responsible for the full fee at the time of service. Dr. Burgee will provide an Insurance form with procedure (CPT) and diagnosis codes once the evaluation and results are completed for you to submit to your insurance for consideration of reimbursement.

Dr. Burgee recommends that you contact your insurance company directly to inquire about possible partial reimbursement based upon your policy, deductibles and coverage. Below are tips when talking to your insurance company.

Document all telephone calls with the date, time and name of representative

Ask the following questions:

• Do I have mental or behavioral health policy that includes Psychological Assessment/Testing?
• Does my policy include out-of-network benefits for this service?
• What is the deductible for out-of-network mental health/psychological testing?
• Is prior authorization required?
• Is a referral from my primary care physician required?
• Does my policy cover the following CPT codes: 96136, 96137, 96130 or 96131?
• Does my policy cover testing/assessment for any of the following conditions:
  ADHD, Psychological Disorders (e.g., anxiety, depression, adjustment disorders) and/or Learning Disorders (e.g., dyslexia)?
